Official Policy of The Golden Torch & Split Does Work, LLC
This policy outline is hereby the living document used to inform, protect, and dictate the operations of The Golden Torch as a highly functioning and professional body arts studio under the ownership of Split Does Work, LLC. Decisions to operate outside of the policies outlined here are at the sole discretion of the business owner, as exceptions to be judged by individual circumstance.
REQUIREMENTS FOR SERVICE
PROOF OF AGE AND IDENTITY
Individuals over the age of 18 seeking body modification services like tattooing or piercing must provide valid proof of age and identity. Acceptable forms of ID include Drivers Licenses or other state-issued photo IDs, passports, or military IDs that include the individual’s photograph, name, and birth date. Physical IDs and/or documents must be present at each instance of service.MINORS WITH PARENTAL CONSENT
Individuals under the age of 18 may receive body modification services with the written consent of a biological parent or legal guardian. The required documents are as follows: state-issued photo ID or school ID of the minor, state-issued photo ID of the parent or guardian, and the birth-certificate of the minor baring the name of the present biological parent. In the case of legal guardianship, the birth certificate can be substituted with legal court documents declaring guardianship.
CONSENT AND WAIVER FORM
Individuals are required to complete a form waiving the studio of liability and consenting to the procedure. One form covers any and all procedures performed on that day, but expires when the client leaves the studio. Another form must be completed on each future visit.ORDERLY CONDUCT
Individuals seeking services are expected to conduct themselves in an orderly manner. Individuals who are obviously under the influence of drugs or alcohol may be declined service, as may others who cause alarm to practitioners or clients present at the studio. Individuals demonstrating an inability to make rational decisions, especially those regarding permanent modifications to the body, will not be serviced.

GOOD FAITH ADVICE
Tattoo Artists at The Golden Torch are taught to give their best advice to clients. To the ends of the artist’s best vision of the client’s tattoo, he or she will always give honest feedback and make suggestions in the interest of the tattoo’s composition, style, longevity, and taste. It’s ultimately the client who will decide the particulars of the design, but if the problems with the concept or design are significant, an artist may decline to perform the tattoo.WALK-INS
Individuals are encouraged to come to the studio if they hope to get tattooed on the spot. Speaking with an artist in person is the best way to determine if a tattoo can be applied right away or if an appointment should be made. Walk-in tattoos are first come; first served.APPOINTMENTS
Appointments can be made in person or from this website, but not over the phone.DEPOSITS
